As of September 2026, Tabflows' DPC Finder has hand-mapped 2,663 real direct primary care and concierge practices across all 50 states plus Washington, DC. That's not a scraped or estimated figure, it's a real count of individually verified listings, each with a name, city, state, and model, and most now with an address, phone number, and provider on file too.
Numbers like this move constantly. Practices open, close, and get missed, and any directory (ours included) is a snapshot of a moving target, not a permanent census. Treat this as "here's what we can verify right now," not a settled fact for all time.
The breakdown by model
Not every practice that calls itself DPC runs the same way. Our dataset sorts each one into a model:

Pure DPC practices don't bill insurance at all. You pay a flat monthly or annual membership fee directly, and that's what covers your visits.
Hybrid practices run a DPC-style membership alongside a traditional insurance-billed practice, often the same doctor doing both.
Onsite clinics are employer- or union-sponsored, usually restricted to that employer's own employees and their families rather than open to the public.
Unknown/other is exactly what it sounds like: practices where our source data didn't clearly state which model they run.
Where this number comes from
We built this count from Tabflows' own sales-research CRM, a real, proprietary dataset the team has been maintaining, not scraped from anyone else's directory. Every practice was cross-referenced against the US Census Bureau's public Gazetteer and address-level geocoding APIs to place it on a map accurately. Why the count keeps changing
DPC is still a young, fast-growing model. New practices open, a few close, and details like phone numbers and providers change as staffing does. That's true of every directory in this space, not just ours, so whatever number you're looking at, including this one, is worth treating as a current best estimate rather than a fixed fact.
