Track every specialist referral until the report is reviewed

Each referral is an owned Task with a return date, so a missing report surfaces and closing it means the provider read it.

The situation

A referral feels finished the moment it goes out. It is not finished until the specialist's report comes back and a provider reads it. Most clinics track the sending well and lose the second half, because nothing prompts anyone to check when the report does not show up.

A referral can stall at three points. The specialist's office never got it, the patient never booked, or the visit happened and the report sat unread. From your side, all three look the same: silence.

Steps

  1. Make each referral a Task the day it goes out Use the patient-page Task button so the patient is linked, and add a clinic Tag such as Referrals. Set Due on to the date you expect the report back, such as two to three weeks after sending, or one week after the specialist visit, so the check is set before anyone has to remember it.
  2. Give the follow-up to a coordinator, not the provider In Assigned to, choose your referral coordinator or the front desk teammate who owns referrals. The provider should hear about a referral only when its report needs review.
  3. Track the three checkpoints In Description, add checklist items for sent, scheduled and report received, then one for provider review. Type @ and the ordering provider's name inside that last item, and leave the Task open until it is checked.
  4. Keep one list of open referrals Filter Tasks by the Referrals Tag and the To do status, which includes Waiting, and each teammate who works referrals saves it under Favorite filters. They see which referrals are still out without opening each chart.
  5. Call when the date comes and nothing is back Set the status to Waiting while you wait on the specialist. When the Task surfaces in My Tasks with no report, call the specialist's office and record what you learned in Comments.
  6. Stop waiting on a stuck referral When a referral has been open for two months with no scheduling confirmation and no report, call the patient and note it in Comments. Then @ mention the provider about a new referral to a different specialist.

The outcome

Every referral has an owner and a return date, so an overdue one shows up on a shared list instead of waiting for the patient to ask. Closing the Task means a provider read the report, not just that a fax landed somewhere, and no patient is left assuming a referral is moving when it stopped weeks ago.
