Keep advance directives and POLST forms findable and current

Every patient's directive status is logged, the documents live in one known place, and reviews come up at the visits that matter.

The situation

Advance directives and POLST forms sit unused most of the time. Then, in a crisis, an emergency visit or a rapid decline, they have to be found right away, and slow retrieval means care that may not match what the patient wanted.

The conversation is easy to postpone in a packed visit, so across your panel some patients have a directive on file and many do not, and nobody knows which without going to look. Even a filed document can be hard to find if only the person who handled it knows where it went, and one completed years ago may no longer reflect what the patient wants.

Steps

  1. Pick one place for the documents Agree where every advance directive and POLST is filed in your chart, then have an Admin add that rule to Practice Brain as a short staff workflow. Any teammate can then ask Sidekick where to look, instead of depending on who filed it.
  2. Log every patient's status, including none Give each patient a Task with a clinic Tag such as Advance directive, which an Admin creates once in Task Settings. In Description, write whether a directive or POLST is on file and when it was completed, or that there is none yet, and set Due on for the patient's next annual visit.
  3. Review at the natural touchpoints At the annual visit, ask whether the document is still accurate, or start the conversation if there is none. After a new diagnosis or a hospitalization, move Due on up to the next visit and say why in Comments.
  4. Flag the patients who need it most For a seriously ill or high-risk patient, select Mark as urgent and move Due on up, so their document is reviewed sooner than the annual visit, especially when it is missing or out of date. The red Urgent marker sets it apart from routine reviews.
  5. Replace the old version cleanly When a patient completes a new document, file it in the agreed place, update the Task's Description with the new date, and note in Comments which version it replaces. Nobody should have to guess which one is current.

The outcome

Anyone on your team can tell whether a patient has a directive and where to find it, without relying on who handled that patient's paperwork. Documents get reviewed at the visits where they can change, and when the moment comes, which is rarely a convenient one, the right document is found fast.
