Catch duplicate records and entry errors before they cause a mix-up

A scheduled review finds near-matches and bad entries, and each one is tracked as a Task until it is fixed.

The situation

Your EHR, billing system and messaging tool each hold the same patient, entered by different people at different times. A nickname in one, a typo in a phone number in another, a returning patient given a new record because nobody searched first. None of it causes trouble the day it happens.

It adds up quietly, and nobody's job is to look for it. A duplicate that lives partly in the EHR and partly in billing is not visible from either system alone. You find out when a message goes to the wrong record, a provider opens a chart missing half its history, or a claim bounces because the insurance on file does not match the payer's.

Steps

  1. Put the review on a schedule Create a Task for one owner, often your office admin, and set Repeat to Monthly, or to Custom at 90 days for a quarterly check. The check then happens before something goes wrong, not after.
  2. Check the fields that cause mix-ups In the review Task, add checklist items for the fields that matter most: name, date of birth, phone number and insurance. Look for near-matches, such as similar names, matching birthdates or similar phone numbers and addresses.
  3. Make each find its own Task For every duplicate or bad entry, create a Task with the Patient set, what is wrong in Description, and a clinic Tag such as Records. Each one is then tracked to a fix instead of noted and forgotten.
  4. Fix it without losing either version Before you merge a duplicate or correct an entry, compare both versions so nothing from either one is lost, then make the change in the system that holds the record. Note what you merged or corrected in a comment, then set the Task to Done.
  5. Search before you create In Task Settings, under Templates, add a first checklist item to your new-patient intake task template: search for an existing record before creating a new one. Returning patients then get matched instead of duplicated.

The outcome

Drift gets caught while it is small, every duplicate and entry error has an owner and a record of how it was fixed, and fewer new duplicates start at intake. Your patient data stays trustworthy as the panel grows instead of piling up small errors that eventually cause a real mix-up.
