Close the loop on every patient call that needs a follow-up

Each call that creates work becomes a patient-linked Task with an owner, a due date and a status that stays open until it is done.

The situation

Someone called, someone wrote it down, someone forwarded the message. That feels like progress, but the patient still needs an answer, a review, a callback, a schedule change or a refill update. The message is only evidence that work exists.

Small clinics lean on trustworthy people to keep this moving, and it works until the day gets full. The front desk remembers three callbacks, the MA remembers a refill question, the doctor remembers a lab call. Then the patient calls again and the team rebuilds the story from memory. That is a visibility problem, not a people problem.

Steps

  1. Make each call that creates work a Task Use the patient-page Task button so the Task is linked to the patient. In Description, write the request itself: not "Jane called" but "Jane asked about her lab follow-up and needs provider review."
  2. Send it to the right bucket Administrative calls go to the teammate or group who can answer or schedule, and clinical questions go to the provider for review. Refill calls get a Refills clinic Tag for whoever handles refills, with the chart context they need in Description, and unclear calls go to one person for triage with enough detail to decide what happens next.
  3. Set when the patient should hear back Set Due on to the day of the callback, so the Task surfaces in the owner's My Tasks then. When it needs attention sooner, move Due on up and select Mark as urgent.
  4. Keep the story on the Task Record each attempt, voicemail and handoff in Comments. When the next step waits on a provider or the patient, set the status to Waiting, and @ mention anyone whose input you need without changing who owns it.
  5. Close it only when the patient has their answer Set the Task to Done when the outcome is finished and no follow-up remains. When the same patient calls again, filter Tasks by Patient to see what is already open before you create anything new.

The outcome

Every call that creates work has a patient, an owner, a due date, a status and enough context for the next person to act, whichever channel it came through. Follow-through stops depending on who remembers, and when a patient calls again, the story is already on the Task.
