## The situation

A school or camp form is not hard on its own: pull an immunization record, do a physical, sign. The trouble is timing. Most families ask in the same few weeks before school or camp starts, right when the return-to-school schedule is already full.

Requests arrive informally, as a call, a note or a form left at the front desk. In the surge, whoever is loudest gets handled first, physicals get squeezed in, and families who asked weeks ago call anxiously as their deadline gets close. Nobody can see what is still outstanding and what is done. Most families wait until close to the school year or camp start date to ask, so requests bunch into a short window, and a first-come queue can leave a family who asked early waiting as long as one who asked at the last minute.

## Steps

1. **Log each request with its deadline and form** However it arrives, make it a Task on the student's patient page with a clinic Tag such as Forms. In Description, write what is needed, such as an immunization record, physical exam or sports clearance, the deadline, and which form the school or camp requires, and attach that form when the family provides it.
2. **Set Due on by the deadline, not the request date** Choose a Due on early enough to finish before the deadline. A form due in three days then surfaces before one due in three weeks, no matter which family asked first.
3. **Keep provider time for the parts that need it** Assign the Task to the front desk or office teammate who handles forms, and add checklist items for each part. Type @ and the provider's name only inside the exam, clearance or signature items, so the paperwork stays with the front desk.
4. **Keep one view of what is still open** Each teammate working the forms saves the same filter for the Forms Tag and the To do status under Favorite filters. During the surge, everyone sees what is still outstanding.
5. **Remind families before the next rush** Ahead of the season, schedule an @Sidekick request in My chat to search Tasks across the clinic for last year's school and camp forms. Reach out early to those families and to any whose school calendar you know, so some requests come in before the crunch.

## The outcome

The seasonal surge becomes a sorted list you can work through in deadline order, with provider time kept for exams and signatures. Families who asked early are not left waiting behind the loudest callers, and a predictable crunch becomes something the practice can plan for.
