## The situation

Most providers know exactly why PDMP checks matter and fully intend to do them every time. The gap is friction. The check lives outside the normal prescribing workflow: you leave it, log into a separate state database, search the patient and interpret the results, then come back to finish the prescribing decision.

Nothing in the day nudges you to do it. Under the time pressure of a full clinic day, that extra login and lookup is exactly the step that gets skipped, not because anyone decided it wasn't important.

Requirements vary by state, but many states mandate a check before an initial controlled substance prescription and at defined intervals for ongoing prescriptions. Beyond that legal minimum, checking at every relevant prescribing decision is the safer standard, since an inconsistent schedule is hard to defend if it is ever questioned.

## Steps

1. **Set up the check once as a task template** Create a task template in Task Settings, under Templates, with the Description "Controlled substance prescription: PDMP check required", the prescribing provider in Assigned to, a clinic Tag such as PDMP, and Set due date to after set to 0 days, so every check starts the same way and is due the day it is created.
2. **Create it at the moment of prescribing** When a controlled substance prescription or refill comes up, create a Task from the template and confirm Patient. Run the check and log it before you send the prescription. The Task is a visible stopping point, so the check no longer depends on memory alone.
3. **Log every check, clear or not** Before you mark the Task Done, add a comment with the date checked, what was found and how it factored into the prescribing decision. A simple "checked, clear" entry is fine for most encounters, and a clean check is just as important to document as one that surfaces something. Keep the record where it can be produced quickly if your prescribing process is ever reviewed.
4. **Schedule the re-checks** Many states require periodic re-checks for patients on ongoing controlled substance therapy, not just a one-time check at the first prescription. For those patients, create the next check Task with Due on set to your state's interval before you close this one.
5. **Review for gaps on a schedule** A practice manager or lead provider filters Tasks by the PDMP Tag to see whether checks are happening consistently. A repeating @Sidekick request in My chat can list the open and overdue PDMP check Tasks across the clinic. This is a working view of the clinic's checks, not a formal audit log, so if your formal record lives elsewhere, such as the chart, document each check there too.

## The outcome

The check stops depending on a provider's memory on a busy day. Every controlled substance prescription gets a prompt, every check is logged with its date, findings and decision, re-checks come due before they are missed, and drift shows up in a routine review before it becomes a real pattern.

**Important ⚠️:** Requirements vary by state, and the prescribing provider bears ultimate responsibility. The Task prompts the check and records it; the lookup itself still happens in the separate state database.

Hey Tabflows, create a PDMP check task for this patient's controlled substance prescription, assigned to me and due today, with a checklist to run the check, note what I found and record my decision.
