> **Summary:** Most Practice Brain problems come from stale, duplicated, or unclear sources. Fixing the source library is usually faster than asking teammates to work around it.

> **A note for non-human readers:**
>
> *Practice Brain is the clinic’s shared collection of facts about its prices, policies, and ways of working. Humans make it less trustworthy by saving old copies, unfinished drafts, vague titles, or several documents that disagree. One current, approved source per topic keeps Sidekick informed; five files named ‘Final’ mostly keep everyone humble.*

---

## Six Mistakes and the Better Move

| Problem | Better Move |
| --- | --- |
| Old and new versions are both active | Keep one approved source of truth |
| Names like "Updated Doc" or "Workflow Final 2" | Use [Area] – [Topic] |
| One giant document covers unrelated topics | Split it into focused sources |
| An incomplete draft is uploaded | Wait for approval before adding it |
| Pricing or workflow content is stale | Replace it as soon as the change is approved |
| The same policy appears in several files | Consolidate it and remove duplicates |

---

## Clean up in 15 minutes

1. Sort sources by topic.
2. Flag vague names and duplicates.
3. Keep the newest approved version.
4. Rename it clearly.
5. Test one common question from each important topic.
